Aging in Place Oregon provides home safety assessments and recommends home modifications to help older adults stay safe, independent, and stable in their homes—especially after hospitalization.

Our goal is to help your patients return to a home that’s safe and ready for their next step to recovery – and to help reduce the chance of readmission.
